Participant Waiver and Declaration

By participating in this Goalball Come & Try Day, I acknowledge and agree to the following:

1. Assumption of Risk: I understand that participating in Goalball involves physical activity and inherent risks, including the risk of injury. I voluntarily choose to participate and accept all risks associated with the activity.
2. Release of Liability: I agree that the Victorian Goalball Association will not be held liable for any injury, loss, or damage sustained while participating in this event, except where required by law.
3. Medical Fitness: I confirm that I am medically fit to participate in this activity and will notify the Victorian Goalball Association of any relevant medical conditions or injuries that may affect my participation.
4. Following Instructions: I agree to follow all instructions, rules, and safety guidelines provided by the Vicotorian Goalball Association  at all times to ensure a safe and enjoyable experience for everyone.
5. Personal Responsibility & Conduct: I agree to behave respectfully and responsibly during the event and understand that failure to do so may result in my removal from the activity.
